Compliance forms for all divisions must be filled out annually.
MEMBERSHIP NOT VALID WITHOUT COMPLETED FORM & PAYMENT.
NCSLMA offers the convenience of paying either by credit card or invoice. Invoice payments are due within 14 days of completing the form. Membership is not active until payment is received. In accordance with the NCSLMA By-laws, all memberships that remain unpaid will lapse on their renewal date. Membership fees are non-refundable, and over payments will be credited toward future dues.
